You can apply at a docomo Shop. If you apply by phone, PC, i-mode, or sp-mode, you will be sent an application form by post to fill in and then send back.

An application form (in Japanese only) will be sent to you by post. Please complete the form and send it back. The required docomo ID and password can be easily issued, so even beginners can start using right away.

Application Start Time
- Hearty Discount takes effect from the day of application (applicable charges billed the following month).
- Discounts for dialing and data communications charges take effect from the time of application.
Notes on Application
Items required
Original copy of one of the following documents: Government-issued identification booklet for the physically challenged (Shintai Shogaisha Techo), government-issued identification booklet for the mentally challenged (Ryoiku Techo), or government-issued welfare booklet for the mentally ill (Seishin Shogaisha Hoken Fukushi Techo)
(When an application is made by post, copies of the necessary documents will be accepted.)
When the subscriber and user are the same person and an application for Hearty Discount is made at the same time as a new application, if the address shown on the document is different from the applicant's current address, a utility receipt (utility bill with receipt stamp or automatic bank account transfer notice with an issuing date [transaction date]), Certificate of Residence (Juminhyo), or Certificate of Registered Matters on Alien Registration issued within the last 3 months will need to be submitted. In such case, the documents must be under the applicant's name.
Minors (when applying for Hearty Discount at the same time as applying for a new subscription)
A letter of consent of legal parent or guardian
2(bearing a legal parent or guardian's signature or seal) is required in addition to the above.
If application by the subscriber is difficult
Application by a representative is accepted.

Documents to verify identity of representative
An original copy of one of the following documents is required.
Driver's licence under the representative's name, Japanese passport, National Health Insurance Card (all must be valid), etc.
If changes have occurred in the booklet
In the following cases, call the docomo Information Center at 0120-005-250 (toll free), or visit a local docomo Shop.
- If details (identification number and issuing body) have been changed
- If you are no longer qualified and have returned your identification booklet
