Dialing, communications, or other charges may apply separately to use individual services.

• Subscription to i-mode or sp-mode is required to use Melody Call.
• You cannot apply for Option Pack Discount if you are already subscribed to FOMA Data Communications Billing Plan1, FOMA Ubiquitous Plan, Type Limit, Type Limit Value, Family Wide Limit, Family Wide Limit Value, Limit Plus2, Do Nich Yo3, Pre-Call4, or Hearty Discount.
• If you cancel any one of Melody Call, Voice Mail, Call Waiting, or Call Forwarding, your subscription to Option Pack Discount will be automatically cancelled; and regular monthly charges will apply from the day of cancellation (on a per diem basis).
• If you are already subscribed to Melody Call Enjoy Course and then apply for Option Pack Discount, the Melody Call Enjoy Course subscription will continue. If you wish to use Basic Course instead, you will need to change your subscription from Enjoy Course to Basic Course.

• 1 New applications for FOMA Data Plan 22 are no longer accepted as of May 31, 2005.
• 2 New applications for Limit Plus are no longer accepted as of October 31, 2005.
• 3 New applications for Do Nich Yo are no longer accepted as of December 31, 1999.
