NTT DOCOMO, INC. shall treat personal information relating to its shareholders (hereinafter referred to as "Shareholder Personal Information") in accordance with the policies set forth below.
For the purpose of this Policy, the term "shareholder" means those shareholders and registered share pledges or their legal representatives who are individuals listed or recorded in the register of shareholders.
1-1. Use of Shareholder Personal Information by NTT DOCOMO
NTT DOCOMO may use Shareholder Personal Information for the following purposes:
- For the exercise of rights or performance of duties in accordance with the Corporation Law;
- To provide various benefits from NTT DOCOMO to Shareholders in their capacity as Shareholders;
- To implement various measures for an efficient and effective relationship between Shareholders and NTT DOCOMO, from the perspectives of a corporation and its members; or
- For shareholder management such as collecting shareholder data in accordance with standards set by applicable laws and regulations.
Provided, however, that the foregoing shall not apply in any of the events specified in the provisions of Article 16, Paragraph 3 of the Personal Information Protection Law.
When NTT DOCOMO receives Shareholder Personal Information in writing directly from shareholders, it shall state clearly the purposes for which such information shall be used, except in those instances specified by the provisions of Article 18, Paragraph 4 of the Law Concerning the Protection of Personal Information (hereinafter referred to as the "Personal Information Protection Law").
1-2. Provision of Shareholder Personal Information to Third Parties
NTT DOCOMO will not provide Shareholder Personal Information received from Shareholders to third parties, except in those cases as specified by the provisions of Article 23, Paragraph 1 or 4 of the Personal Information Protection Law.
1-3. Joint Use of Shareholder Personal Information
NTT DOCOMO will not engage in joint use with any particular third parties of Shareholder Personal Information received from Shareholders.
1-4. Procedures for the Disclosure of Shareholder Personal Information in NTT DOCOMO’s Possession
NTT DOCOMO shall take necessary measures with respect to requests from its shareholders for disclosure, correction, or termination of use of Shareholder Personal Information in its possession which are submitted in accordance with the following procedures.

In order to make a request concerning Shareholder Personal Information, Shareholders are requested to download and fill in the relevant request form, attach any additional necessary documents, and mail them to NTT DOCOMO.

Please note that the foregoing shall not apply in the case of requests for disclosure in accordance with provisions of the Corporation Law or any other applicable laws and regulations.
Please also note that, NTT DOCOMO will not correct, terminate the use of, or delete, Shareholder Personal Information listed or recorded in the Registers of Shareholders except in accordance with the procedures for sale of NTT DOCOMO shares by shareholders pursuant to the provisions of the Corporation Law.

Treatment of Shareholder Personal Information Received in Relation to Requests concerning Personal Information
Shareholder Personal Information received in relation to requests concerning personal information shall be used only to the extent necessary to process the request.
Method of Response
A response shall be sent to the requesting party (the Shareholder) or his or her representative by restricted delivery postal mail.

NTT DOCOMO shall comply with the Personal Information Protection Law and other applicable laws and regulations, and shall observe guidelines issued by competent government ministries and agencies and industry guidelines with respect to the treatment of Shareholder Personal Information.
NTT DOCOMO shall implement appropriate organizational security measures, personnel security measures, physical security measures, and technological security measures with respect to the treatment of Shareholder Personal Information.
NTT DOCOMO shall periodically, continuously, and flexibly review management systems and internal regulations, shall continuously improve management systems in conjunction with changes in technology and legal systems, and shall actively protect Shareholder Personal Information.
