Flow of application for usage registration

1.Request a registration application form via i-mode/sp-mode or by phone. 2.DOCOMO sends the application via postal mail. 3.The applicant sends back the completed application form together with a copy of an identification document. 4.After DOCOMO checks the details of the application, service registration is completed. After service registration is completed, a Message R will be sent to notify the applicant of this.

  • noticeTo use this service, the user must be 20 years of age or older. Application forms can be requested by anyone, but the service cannot be used if the applicant is under 20 years of age.
  • noticeOnly the line subscriber can apply.
  • noticeOnly the phone number of the mobile phone from which the application form was requested can be registered.

Sending back the registration application form and copy of identification document

Please fill in all the required items on the application form and send it back together with a copy of a document verifying your identity.

The following identification verification documents are accepted.

Japanese nationals Japanese driver's license Must be issued by a prefectural or metropolitan Public Safety Commission. (International driver's licenses are not accepted.)
Japanese passport -
Foreign nationals Certificate of Alien Registration Must have a valid status of residence and show at least 90 days until the period of stay expiry date.
Residence Card
Special Permanent Resident Certificate With in an effective period.
  • noticeIdentification documents are accepted on the conditions that they include a photo of the applicant's face and show the applicant's address, and that the document itself is valid. The conditions for identification verification documents for this service differ from those for new mobile phone subscription.
  • noticeThe name and address written on the usage registration application form must match both the subscriber name and address of the mobile phone line and the name and address shown on the identification document.
  • noticeOn the usage registration application form, write the phone number of the mobile phone from which the application form was requested.

Completion of registration

After registration is completed, you will be notified via Message R.

  • noticeApplication reviews are based on predefined stipulation of DOCOMO.
  • noticePlease note that it will take some time to complete the registration after the application form is returned to DOCOMO.

You can use this service after you receive a Message R informing you that registration is complete.
