How to Apply for Usage Registration

A usage registration application form can be requested via the i-mode site or by phone.

How to apply		Business hours
sp-mode	Application can be made via the docomo Money Transfer site on sp-mode. A usage registration application form can be requested from (Another window will open.)here. Access is only available from a DOCOMO sp-mode compatible model. Your network PIN is required.	24 hours a day
i-mode	Application can be made via the docomo Money Transfer site on i-mode. Your network PIN is required.	24 hours a day
Phone	docomo Information Center From DOCOMO mobile phones 151(toll free, in Japanese only) Can only be called from DOCOMO mobile phones. From landline telephones and mobile phones 0120-005-250 May not be accessible from some IP phones. 9 a.m. to 8 p.m. (365 days a year) Applications for various orders, suspending/resuming service due to loss, etc. are also accepted. After a connection is established, follow the voice guidance to select a number. You can even perform the skip operation while the voice guidance is playing. For details, refer to the Voice Guidance Information page. Voice Guidance Information	Open from 9 a.m. to 8 p.m.

• To use this service, the user must be 20 years of age or older. Application forms can be requested by anyone, but the service cannot be used if the applicant is under 20 years of age.
• Only the line subscriber can apply.
• Only the phone number of the mobile phone from which the application form was requested can be registered.

Please fill in all the required items on the application form and send it back together with a copy of a document verifying your identity.

The following identification verification documents are accepted.

• Identification documents are accepted on the conditions that they include a photo of the applicant's face and show the applicant's address, and that the document itself is valid. The conditions for identification verification documents for this service differ from those for new mobile phone subscription.
• The name and address written on the usage registration application form must match both the subscriber name and address of the mobile phone line and the name and address shown on the identification document.
• On the usage registration application form, write the phone number of the mobile phone from which the application form was requested.

After registration is completed, you will be notified via Message R.

• Application reviews are based on predefined stipulation of DOCOMO.
• Please note that it will take some time to complete the registration after the application form is returned to DOCOMO.

You can use this service after you receive a Message R informing you that registration is complete.
