• Your current FOMA handset (including DOCOMO UIM Card (FOMA Card)) (except in cases where your phone has been lost or stolen12)
  • 1 Originals of documents verifying the subscriber's identity are required when the phone has been lost or stolen.
  • 2 A handling fee must be paid for reissuing a DOCOMO UIM Card (FOMA Card) when the phone has been lost or stolen (handling fee or reissuing a DOCOMO UIM Card (FOMA Card): ¥3,150).
  • You are required to enter your network PIN, provide documents verifying identity, show the (Phone purchase confirmation site [your network PIN must be entered]) (in Japanese only) on your current mobile phone, etc., so we can confirm your identity.
  • At docomo Shops, the (Phone purchase confirmation site) cannot be used to confirm your identity. Also, at DOCOMO dealers other than docomo Shops, confirmation by entering your network PIN cannot be made.
  • If a representative applies on behalf of an organization or other individual (application by proxy), the representative may need to present proof of identity, etc. If the official identification of the person who applies is a Japanese Health Insurance Certificate, a utility receipt, credit card, student ID, Certificate of Residence (Juminhyo), or Certificate of Registered Matters on Alien Registration does not need to be attached.

  • The application must be made by the subscriber in order to use DOCOMO installment payments under a personal name. An application cannot be accepted from a representative (except for a legal representative or family member of the subscriber).
  • When applying for a DOCOMO installment payment contract (individual credit purchase brokerage contract), monthly installment payments (individual credit purchase brokerage contract/installment sale contract) should be made by automatic account transfer or credit card.

FOMA service subscribers can select Value Course or Basic Course when purchasing an applicable handset model.

• When changing mobile phone models, please purchase a new battery, battery charger, and other accessories if your current ones are not compatible with the new model.
• Refer to docomo Points Service to learn how you can use points that are earned when paying your monthly DOCOMO bill in full.
• Data such as phone numbers and mail addresses can be backed up through docomo Welcome Support.
• If you select to purchase a mobile phone in installments, you will need to apply for a separate DOCOMO installment payment contract (individual credit purchase brokerage contract).   About Purchasing by Installment Payment
• If you purchase a new mobile phone before completing the promised period of use after purchasing a mobile phone with Basic Course, you will be required to pay a Basic Course cancellation charge. That charge will apply for the number of months remaining in the promised period of use, and the charge will be ¥630 multiplied by the number of months remaining.
