Documents Verifying the User’s Identity

Please include one of the following sets of documents in the same envelope when you submit an Application for Disclosure of Personal Information.

Individual
(Applicant)
  • Seal Registration Certificate (Original copy issued within the last three months with the User’s current address.) notice1

  • User’s Seal Registration Certificate (Original copy issued within the last three months with the subscriber’s current address.) notice1
  • Representative’s Seal Registration Certificate (Original copy issued within the last three months with the subscriber’s current address.) notice1
  • noticeThe subscriber himself/herself must fill out the authorization section on the Application for Disclosure of Personal Information.
  • noticeDOCOMO may call the contact information for verification.
Individual
(Legal Representative)
  • Documents that verify that the person is the legal representative. [Original copy issued within the last three months.]
    e.g. If the subscriber is a minor (under 20 years old): Official copy of the Family Register (Kosekitohon) that proves parent-child relationship.
    e.g. If the subscriber is an adult (20 years old or over) and has a legal guardian: Certificate of Registered Matters (Tokijikoshomeisho) that notes the legal guardian’s name.
    e.g. If the subscriber is a minor (under 20 years old) and has a legal guardian: Official copy of the Family Register (Kosekitohon) that notes the legal guardian’s name.
  • Legal representative’s Seal Registration Certificate (Original copy issued within the last three months with the representative’s current address.) notice1
Company
  • Seal Registration Certificate (Original copy issued within the last three months with the company’s current address.)
  • Valid copy of employee ID or other similar document or a business card.
  • Employee’s Seal Registration Certificate (Original copy issued within the last three months with the employee’s current address.) notice1
  • noticeDOCOMO may contact the company (e.g. General Affairs Department) to for verification.
  • notice1 For (1) only, a copy of one of the following documents that verify identity will be accepted for subscribers without a Certificate of the Registered Seal.
    However, an original of a Certificate of Residence (Juminhyo) or Certificate of Registered Matters on Alien Registration must be presented.
Japanese
Driver’s License

Must be valid and issued by a Prefectural Public Safety Commission.
(International driver’s licenses are not accepted.)

Japanese Passport

Must be valid and show the applicant’s current address.

  • Student ID must be valid and show applicant’s name, date of birth, and photo or address.
  • Certificate of Residence (Juminhyo), Certificate of Registered Matters on Alien Registration, and utility receipt must be issued within the last three months and show the applicant’s current address. (Only utility receipts for electricity, gas, water, or NTT East/West telephone are accepted.)

Notes

  • noticeCopies of Basic Resident Registration Card or Japanese Health Insurance Certificate are not accepted.
  • noticeIf the address on the official identification is different from the applicant’s current address, a Certificate of Residence (Juminhyo) which verifies the current address or a utility receipt showing the applicant’s current address (only electricity, gas, water, NTT East/West telephone receipts) must be attached (all the above documents must have been issued within the last three months).
  • noticeDOCOMO will send a reply to the subscriber by post (to either the subscriber’s address or the billing address). However, in the case of an application from a legal representative, the reply will be sent to the legal representative’s address (the address entered on the official identification of the legal representative). In the case of an application from a company, the reply will be sent to either a section of the company such as the General Affairs Department or to the company employee who applied for the information (the company address or billing address). Replies can only be received according to ID verification procedures of Japan Post Service.
