Documents Verifying the User’s Identity
Please include one of the following sets of documents in the same envelope when you submit an Application for Disclosure of Personal Information.

1 For (1) only, a copy of one of the following documents that verify identity will be accepted for subscribers without a Certificate of the Registered Seal.
However, an original of a Certificate of Residence (Juminhyo) or Certificate of Registered Matters on Alien Registration must be presented.

Notes
Copies of Basic Resident Registration Card or Japanese Health Insurance Certificate are not accepted.
If the address on the official identification is different from the applicant’s current address, a Certificate of Residence (Juminhyo) which verifies the current address or a utility receipt showing the applicant’s current address (only electricity, gas, water, NTT East/West telephone receipts) must be attached (all the above documents must have been issued within the last three months).
DOCOMO will send a reply to the subscriber by post (to either the subscriber’s address or the billing address). However, in the case of an application from a legal representative, the reply will be sent to the legal representative’s address (the address entered on the official identification of the legal representative). In the case of an application from a company, the reply will be sent to either a section of the company such as the General Affairs Department or to the company employee who applied for the information (the company address or billing address). Replies can only be received according to ID verification procedures of Japan Post Service.
