Service Counters

You can apply at a docomo Shop. If you apply by phone, PC, Smartphone, or docomo Feature Phone, you will be sent an application form by post to fill in and then send back.

You can apply from (docomo Online Application). The required d ACCOUNT can be easily issued, so even beginners can start using right away.

  • When accessing from a PC, a d ACCOUNT is required.
  • When accessing from a smartphone or docomo Feature Phone (sp-mode), a Network PIN is required.
  • Operation procedures on the actual screens are in Japanese.

Application Start Time

Items required

Applying when you already have a DOCOMO subscription

One of the following documents: Government-issued identification booklet for the physically challenged (Shintai Shogaisha Techo), government-issued identification booklet for the mentally challenged (Ryoiku Techo), government-issued welfare booklet for the mentally ill (Seishin Shogaisha Hoken Fukushi Techo), government-issued specified disease treatment recipient certificate (Tokutei Shikkan Iryo Jyukyushasho), government-issued specified disease registrant certificate (Tokutei Shikkan Torokushasho), and specified intractable disease medical expense recipient certificate (Tokutei Iryohi [Shitei Nanbyo] Jyukyushasho)

Application by a representative

The following documents are required in addition to the booklet or certificate.

  • Letter of attorney*2 with contracting customer's signature or written name and seal
  • Documents to verify identity of representative
    (Original copy of a Japanese driver's license, Japanese Health Insurance Card, etc. [must be valid and show the current address])
Applying by post

Send your Hearty Discount application form and a copy of your booklet or certificate.

Applying at the same time as applying for a new subscription for a mobile phone

Show your credit card or bank card for the monthly charge payment procedure in addition to an original copy of your government-issued identification booklet for the physically challenged (Shintai Shogaisha Techo), government-issued identification booklet for the mentally challenged (Ryoiku Techo), government-issued welfare booklet for the mentally ill (Seishin Shogaisha Hoken Fukushi Techo), government-issued specified disease treatment recipient certificate (Tokutei Shikkan Iryo Jyukyushasho), government-issued specified disease registrant certificate (Tokutei Shikkan Torokushasho), or specified intractable disease medical expense recipient certificate (Tokutei Iryohi [Shitei Nanbyo] Jyukyushasho).

If you do not have a credit card or bank card

Your bankbook plus seal and the following additional documents are required.

If the address on the booklet or certificate you will show is different from your current address

The following additional documents are required in addition to the documents required for the charge payment procedure.

  • Utility receipt
    (Utility receipt bearing a receipt stamp or automatic payment notice showing the date of automatic payment)
  • Certificate of Residence (Juminhyo) without Individual Number (social security and tax number) printed on it
If the subscriber is a minor

A letter of consent*2 of a legal parent or guardian (bearing a legal parent or guardian's written name or seal) and an identification document of a legal parent or guardian are required in addition to the booklet or certificate of the subscriber.

Application by a representative

Application by a representative is accepted.
(Only if the representative is a member of the subscriber's family or a legal representative.)

The following documents are required in addition to the booklet or certificate of the subscriber.

  • Letter of attorney*2 with contracting customer's signature or written name and seal
  • Documents to verify identity of representative
    (Original copy of a Japanese driver's license, Japanese Health Insurance Card, etc. [must be valid and show the current address])
  • When a legal representative applies, a letter of consent from the legal representative, identification document of the legal representative, and a document to verify that the person is a legal representative are required in addition to the booklet or certificate of the subscriber.

If changes have occurred in the booklet

In the following cases, call the General Inquiries (docomo Information Center) at 0120-005-250 (toll free), or visit a local docomo Shop.

  • If details (identification number and issuing body) have been changed
  • If you are no longer qualified and have returned your identification booklet/certificate
