Documents Verifying the User's Identity

Please include one of the following sets of documents in the same envelope when you submit an Application for Disclosure of Personal Information.

Individual (Applicant)	Seal Registration Certificate (Original copy issued within the last three months with the User's current address.) *1
Individual (Representative)	User's Seal Registration Certificate (Original copy issued within the last three months with the subscriber's current address.) 1 Representative's Seal Registration Certificate (Original copy issued within the last three months with the subscriber's current address.) 1 The subscriber himself/herself must fill out the authorization section on the Application for Disclosure of Personal Information. DOCOMO may call the contact information for verification.
Individual (Legal Representative)	Documents that verify that the person is the legal representative. [Original copy issued within the last three months.] e.g. If the subscriber is a minor (under 20 years old): Official copy of the Family Register (Kosekitohon) that proves parent-child relationship. e.g. If the subscriber is an adult (20 years old or over) and has a legal guardian: Certificate of Registered Matters (Tokijikoshomeisho) that notes the legal guardian's name. e.g. If the subscriber is a minor (under 20 years old) and has a legal guardian: Official copy of the Family Register (Kosekitohon) that notes the legal guardian's name. Legal representative's Seal Registration Certificate (Original copy issued within the last three months with the representative's current address.) *1
Company	Seal Registration Certificate (Original copy issued within the last three months with the company's current address.) Valid copy of employee ID or other similar document or a business card. Employee's Seal Registration Certificate (Original copy issued within the last three months with the employee's current address.) *1 DOCOMO may contact the company (e.g. General Affairs Department) to for verification.

1 For (1) only, a copy of one of the following documents that verify identity will be accepted for subscribers without a Certificate of the Registered Seal.
However, an original of a Certificate of Residence (Juminhyo) without Individual Number (social security and tax number) printed on it must be presented.

Documents Verifying the User's Identity

Japanese Driver's License	Must be valid and issued by a Prefectural Public Safety Commission. (International driver's licenses are not accepted.)
Basic Resident Registration Card	Must be valid and show the applicant's photo and current address.
Certificate of Alien Registration or Residence card/Special Permanent Resident Certificate	Certificate of Alien Registration must have a valid status of residence and show the applicant's current address. Residence card or Special Permanent Resident Certificate must be valid.
Government-issued identification booklet for the physically challenged (Shintai Shogaisha Techo) or Government-issued identification booklet for the mentally challenged (Ryoiku Techo) or Government-issued welfare booklet for mental illness (Seishin Shogaisha Hoken Fukushi Techo) + Additional document (Utility receipt or Certificate of Residence (Juminhyo) without Individual Number (social security and tax number) printed on it)	Documents must show the applicant's current address. Utility receipt and Certificate of Residence (Juminhyo) without Individual Number (social security and tax number) printed on it must be issued within the last three months.

Notes

If the address on the official identification is different from the applicant's current address, an original of a Certificate of Residence (Juminhyo) without Individual Number (social security and tax number) printed on it which verifies the current address or a copy of a utility receipt showing the applicant's current address (only electricity, gas, water receipts) must be included with the documents that verify identity (all the above documents must have been issued within the last three months).
DOCOMO will send a reply to the subscriber by post (to either the subscriber's address or the billing address).
However, in the case of an application from a legal representative, the reply will be sent to the legal representative's address (the address entered on the official identification of the legal representative). In the case of an application from a company, the reply will be sent to either a section of the company such as the General Affairs Department or to the company employee who applied for the information (the company address or billing address).
