Claim Procedures regarding Retained Personal Data, etc.

DOCOMO will receive inquiries about the contract status of each type of service, viewing and/or issuance of fee statements, and confirmation of transaction history at DOCOMO's customer service desk. Furthermore, the identity of the person that the retained Personal Data is of will be checked at such time.

1. Disclosure and/or Corrections of Personal Data Retained by Telecommunications Businesses

  1. If there is a request for disclosure of retained Personal Data concerning an individual from the person that the retained Personal Data is of, DOCOMO will respond without delay thereto, and sets forth the following procedures. Inquiries regarding the procedures will be handled by DOCOMO's customer service desk:
    1. Application method (application by mail)
      After filling in all of the prescribed matters in the "Personal Information Disclosure Application" (the "Application"), please enclose the identity verification documents and mail them to the below mailing address. The Application is available at the "DOCOMO Information Center." In addition, it can be PDFdownloaded [PDF format:478KB] (in Japanese only) from the home page. Furthermore, please note in red writing, "Application enclosed," on the envelope.

      Please enclose identity verification documents set forth by DOCOMO when applying.
    3. Method of response
      Responses from DOCOMO will be sent to the contracting person (at the address of the contracting person or the address where invoices, etc. are sent) by registered mail (not forwardable).
    4. Fees regarding disclosure
      Fees for disclosure procedures will be required as follows. For basic items of the Application, the fee will be JPY 400 (excluding tax) per telephone number. For items other than the basic items (items entered under "Other Items" of the Application), the fee will be JPY 400 (excluding tax) per item or equivalent to the actual expenses corresponding to the billing contents. Furthermore, the postage fee for the response (for standard sized mail up to 25g: JPY 404) (including tax) will be separately required and will be billed together with the fees for disclosure.
  2. If any of the below items apply, DOCOMO may not disclose all or a part of the retained Personal Data regarding such request. If DOCOMO decides not to make the disclosure, such effect will be notified together with the reason thereof (in such instance, no payment of fees will be required):
    • If the address provided in the Application, the address provided in the identity verification document and the address registered with DOCOMO do not match, or otherwise if the identity of the person cannot be verified;
    • If there is a deficiency in the prescribed Application document;
    • If the subject of the disclosure request does not exist at DOCOMO;
    • If there is the threat of harm to the person or a third party's life, body, property or other right or interest;
    • If there is the threat of causing significant interference to the proper execution of DOCOMO's business; or
    • If it will breach other laws or regulations.
  3. If there is a request for Corrections (corrections, additions, deletions, cease of use or cease of provision to third parties) of retained Personal Data concerning the person, DOCOMO will conduct investigations without delay. In such instance, if the contents of the retained Personal Data regarding such request are not true or if otherwise the handling of such retained Personal Data is found to not be appropriate, the Corrections will be made without delay.     Furthermore, even if there is a request for Corrections, DOCOMO may not make the Corrections for all or a part of the retained Personal Data regarding such request based on the provisions of the laws and regulations, etc.
  4. The requests of (1) ~ (3) above regarding the domestic roaming service and the FOMA specific connection agreement or the Xi specific connection agreement that are provided by DOCOMO will be as follows:
    1. Requests regarding the domestic roaming service provided by DOCOMO will be handled by the telecommunications vendor that has executed a roaming agreement with DOCOMO; and
    2. Requests regarding the FOMA specific connection agreement or the Xi specific connection agreement that is provided by DOCOMO will be handled by the contracting vendor (specific connection vendor) applied for by the customer.
  5. If a notification of the purposes of use of the retained Personal Data is made, DOCOMO will respond by DOCOMO's customer service desk.

2. Disclosure and/or Corrections of Personal Data Retained by Credit Card Businesses, etc.

  1. If there is a request for disclosure of retained Personal Data concerning an individual from the person that the retained Personal Data is of, DOCOMO will respond without delay thereto, and sets forth the following procedures. Inquiries regarding the procedures will be handled by DOCOMO's customer service desk:

  • In the insurance-related services, please inquire with the DOCOMO shop handling the insurance-related services or the below "DOCOMO de Hoken Consultation and Inquiries Center" for inquiries regarding disclosures concerning policyholder information (excluding written disclosures) and disclosures and Corrections of retained Personal Data acquired by DOCOMO prior to the insurance agreement application (the "Pre-Insurance Agreement Personal Information").

      Fees for disclosure procedures will be JPY 400 (excluding tax) per one (1) item or equivalent to the actual expenses corresponding to the billing contents. Furthermore, the postage fee for the response (for standard sized mail up to 25g: JPY 404) (including tax) will be separately required and will be billed together with the fees for disclosure.
      • There is no fee for the disclosure in writing of the Pre-Insurance Agreement Personal Information (excluding postage fees at the time of applying for disclosure).

In such instance, if the contents of the retained Personal Data regarding such request are not true or if otherwise the handling of such retained Personal Data is found to not be appropriate, the Corrections will be made without delay.     Furthermore, even if there is a request for Corrections, DOCOMO may not make the Corrections for all or a part of the retained Personal Data regarding such request based on the provisions of the laws and regulations, etc.
